Specialized pro-resolving mediators (SPMs), including resolvins, protectins, and maresins, are bioactive lipid mediators derived from omega-3 fatty acids. They are involved in actively resolving inflammation and restoring tissue homeostasis. There is emerging scientific interest in SPMs for inflammatory diseases, including Chronic Obstructive Pulmonary Disease (COPD), because chronic inflammation is central to COPD pathogenesis and progression. Preclinical studies in animal models and in vitro experiments suggest that SPMs can reduce neutrophil infiltration, promote clearance of apoptotic cells, and modulate pro-inflammatory cytokine production in the lungs, which could theoretically benefit COPD patients.
However, clinical evidence in humans with COPD is still limited. A handful of small-scale studies and pilot clinical trials have assessed SPM levels in COPD patients or explored SPM analogues, but robust, large-scale randomized controlled trials demonstrating clear clinical efficacy and safety are lacking as of 2024. Thus, while the biological rationale for SPM supplementation is scientifically grounded, and preliminary data are promising, the overall evidence supporting their use specifically for COPD remains preliminary.
In summary, SPMs are not used for COPD on the basis of tradition, but rather due to emerging scientific understanding of their role in inflammation resolution. However, current evidence is at an early stage (rated 2/5), and SPM-based therapies for COPD should still be considered investigational pending further clinical validation.
