Vitamin D has been investigated for its potential role in supporting patients with Chronic Obstructive Pulmonary Disease (COPD), primarily due to its immunomodulatory properties and its involvement in bone and muscle health. Several observational studies have shown that individuals with COPD frequently have low serum vitamin D levels, and deficiency may be associated with worse lung function, increased exacerbations, and higher mortality. A few randomized controlled trials have examined whether vitamin D supplementation can improve outcomes in COPD patients. A notable study published in Lancet Respiratory Medicine (2012) found that high-dose vitamin D supplementation reduced the rate of moderate or severe exacerbations in COPD patients with severe vitamin D deficiency (<10 ng/mL), but not in the overall COPD population.
Systematic reviews and meta-analyses suggest a small benefit of vitamin D supplementation in reducing exacerbation risk in those with baseline deficiency, but there is no strong evidence of improvement in lung function or quality of life for all COPD patients. Current guidelines do not recommend routine vitamin D supplementation for COPD unless deficiency is present, but suggest correcting deficiency as part of overall care. Thus, while there is some scientific basis for use—particularly in deficient individuals—evidence supporting broad use in all COPD patients is modest.
