Butylphthalide, specifically dl-3-n-butylphthalide (NBP), is a compound originally isolated from celery seed and is used in China as a treatment for ischemic stroke. Clinical and preclinical research suggests that Butylphthalide can improve cerebral circulation and protect against neuronal injury. Mechanistically, NBP is thought to enhance microcirculation in the brain, inhibit platelet aggregation, suppress oxidative stress, and reduce inflammation, all of which can support improved blood flow and tissue recovery after cerebral ischemia.
Several randomized controlled trials and meta-analyses have provided evidence that Butylphthalide, when used as an adjunct to standard stroke therapy, may improve neurological outcomes and reduce disability in patients with acute ischemic stroke. For example, a 2019 meta-analysis in Stroke and Vascular Neurology concluded that NBP improved neurological function and activities of daily living in stroke patients. Although most studies have been conducted in China, the consistency of findings and a plausible mechanism of action support its use for cerebral circulation and neuroprotection.
However, despite the promising results, the evidence is less robust outside of China, and more international, large-scale studies are warranted to further validate efficacy and safety. Overall, Butylphthalide's use for supporting brain circulation is supported by substantial scientific evidence, primarily for ischemic cerebrovascular conditions.
