Ligustrazin, also known as tetramethylpyrazine (TMP), is an active compound extracted from the Chinese medicinal herb Ligusticum chuanxiong. It has a long history of use in Traditional Chinese Medicine (TCM) for promoting blood circulation and treating cerebrovascular and cardiovascular diseases. In recent decades, scientific studies—mainly from China—have investigated its pharmacological effects on cerebral circulation. Ligustrazin has been shown in animal and some human studies to have vasodilatory, anti-platelet aggregation, and anti-inflammatory properties, which may contribute to improved cerebral blood flow and neuroprotection.

Clinical trials, including randomized controlled trials, have reported potential benefits of Ligustrazin in the treatment of ischemic stroke and vascular dementia, often as part of combination therapies. However, the methodological quality of these studies is often variable, with issues such as small sample sizes and lack of blinding. Meta-analyses suggest a trend toward benefit, but note the need for higher-quality research and international validation. Ligustrazin is used in China as an injectable or oral medication for cerebrovascular conditions, but it is not widely adopted in Western medicine.
