Fursultiamine is a synthetic derivative of thiamine (vitamin B1) designed to enhance oral bioavailability. Thiamine deficiency is relatively common in patients with chronic liver disease, including cirrhosis, due to decreased dietary intake, malabsorption, and increased requirements. Thiamine deficiency in these patients can lead to serious neurological complications such as Wernicke’s encephalopathy. Therefore, thiamine supplementation—including fursultiamine—has a clear scientific rationale in the context of preventing or treating deficiency states in cirrhotic patients.
A small number of clinical studies and case reports have explored the use of fursultiamine specifically in cirrhosis. These studies indicate that oral fursultiamine is effective at raising blood thiamine levels and may be more efficient than standard thiamine salts in certain cases. However, there is no robust clinical evidence that fursultiamine itself directly improves liver function or alters the course of cirrhosis beyond correcting thiamine deficiency. Guidelines from hepatology societies recommend thiamine supplementation in patients at risk of deficiency but do not specifically endorse fursultiamine over other forms. Thus, while the scientific rationale for use in deficiency prevention or treatment is valid, evidence for direct benefits on cirrhosis itself is limited, justifying a moderate evidence rating.
