Chinese Goldthread (Coptis chinensis) has a long history of use in Traditional Chinese Medicine (TCM), primarily as a bitter herb for "clearing heat" and "detoxifying" the body. In TCM theory, cold sores (herpetic lesions) may be considered a manifestation of "heat toxins" or "damp-heat" in the body. Goldthread, containing the active compound berberine, is often included in traditional herbal formulas aimed at skin eruptions, ulcers, and sores, including those affecting the lips or mouth. However, while this historical use is well documented in classical and modern TCM texts, robust scientific studies specifically evaluating the efficacy of Chinese Goldthread or berberine against herpes simplex virus (the cause of cold sores) in humans are limited. Some in vitro (test tube) research suggests berberine has antiviral and anti-inflammatory properties, but clinical trials confirming its effectiveness for cold sores are lacking. Thus, its use for cold sores is primarily justified by tradition rather than scientific validation, and the overall evidence rating for this purpose is low.
