Licorice root (Glycyrrhiza glabra) has been used traditionally for various ailments, but its use for cold sores (herpes simplex labialis) is supported by some preliminary scientific evidence. The active component in licorice root, glycyrrhizin, has demonstrated antiviral properties in vitro, including activity against herpes simplex virus type 1 (HSV-1), the primary cause of cold sores. Laboratory studies suggest that glycyrrhizin can inhibit viral replication and reduce the ability of HSV-1 to infect cells. Additionally, some topical formulations containing licorice extract have shown promise in small clinical studies, with reports of reduced severity and duration of cold sore outbreaks.

However, the clinical evidence remains limited and comes mainly from small-scale or preliminary trials rather than large, well-controlled studies. The majority of available human data is anecdotal or derived from small studies with methodological limitations. As a result, licorice root cannot yet be considered a standard or primary treatment for cold sores, but the scientific rationale for its antiviral effects means that it is a subject of ongoing research. Safety concerns, particularly with long-term or excessive use of licorice root (due to potential for hypertension and electrolyte imbalance), should also be considered. In summary, licorice root is a promising but not fully validated natural remedy for cold sores, with a moderate but not robust evidence base supporting its use.
