Sophora flavescens, known as "Ku Shen" in traditional Chinese medicine (TCM), has a long history of use for skin conditions, including sores, ulcers, and infections. Its application for cold sores (herpes simplex labialis) comes primarily from this traditional background. In TCM, Sophora flavescens is believed to "clear heat and dry dampness," which is thought to address the underlying imbalances associated with eruptive skin lesions, such as cold sores.

Phytochemical studies reveal that Sophora flavescens contains alkaloids (notably matrine and oxymatrine) with antiviral and anti-inflammatory properties.
