Andrographolide, the primary active compound in Andrographis paniculata, has a history of use in traditional medicine systems in Asia for treating respiratory tract infections, including the common cold. In recent years, scientific studies have investigated its efficacy for this purpose. Systematic reviews and meta-analyses of randomized controlled trials (RCTs) have provided moderate-quality evidence supporting the use of Andrographis extracts (standardized to andrographolide content) for the symptomatic relief of colds, particularly in reducing the severity and duration of symptoms such as sore throat, cough, and nasal symptoms. Proposed mechanisms include anti-inflammatory effects and possible antiviral activity, as demonstrated in in vitro studies, although direct clinical antiviral effects remain less well established.
A 2017 Cochrane review and other meta-analyses concluded that Andrographis was more effective than placebo and comparable to some standard treatments for relieving cold symptoms, with a generally favorable safety profile. However, heterogeneity in product standardization, dosing, and study quality limit the certainty of these findings. While there remains a need for higher-quality trials and standardized preparations, the existing clinical evidence justifies cautious support of Andrographolide-containing extracts for mild to moderate cold symptoms.
