Honey has been traditionally used to alleviate symptoms of the common cold, particularly cough, but there is also some scientific evidence supporting its use, mainly for symptomatic relief rather than direct antiviral activity. Several randomized controlled trials and meta-analyses have shown that honey can be more effective than placebo or no treatment in reducing the frequency and severity of cough in children with upper respiratory tract infections (URTIs). The World Health Organization (WHO) and the American Academy of Pediatrics recognize honey as a treatment option for cough in children over one year of age.

The proposed mechanisms include honey's demulcent effect (coating and soothing the throat), antioxidant properties, and potential antimicrobial activity. Some in vitro studies have shown that certain types of honey, such as Manuka honey, can inhibit the growth of bacteria and some viruses. However, the clinical relevance of honey's direct antiviral effects in the context of common colds remains uncertain, as most studies focus on symptomatic relief.

It is important to note that honey should not be given to children under one year due to the risk of botulism. Overall, while honey does not cure the common cold or directly eradicate viruses, there is moderate scientific evidence supporting its use as a safe and effective option for reducing cough and throat irritation associated with colds in older children and adults.
