Astragalus membranaceus is a traditional herbal remedy widely used in Chinese medicine for the prevention and treatment of the common cold. The use of astragalus for respiratory infections, especially colds, is rooted in its long history as an immune tonic in Traditional Chinese Medicine (TCM), where it is said to "tonify qi" and strengthen the body's defenses against external pathogens. Historically, astragalus has been incorporated into formulas intended to boost immunity and reduce susceptibility to recurrent colds.

Modern scientific evidence supporting its efficacy for treating or preventing the common cold is limited and not robust. Some laboratory and animal studies suggest that astragalus extracts can modulate immune function—enhancing certain white blood cell activities and increasing the production of interferon, a natural antiviral substance. However, high-quality human clinical trials are scarce. A few small studies suggest a possible reduction in the incidence or duration of colds, but these studies often lack rigorous methodology or sufficient sample size.

Overall, astragalus is primarily justified for use in colds by tradition. While there are plausible mechanisms and some preliminary data, the clinical evidence is not strong enough to warrant a higher rating. Most health authorities and systematic reviews conclude that more well-designed clinical trials are needed to confirm its effectiveness.
