Xanthium (commonly known as cocklebur) has a long history of use in traditional Chinese medicine (TCM), where it is known as "Cang Er Zi." It is most frequently included in herbal formulas for the treatment and prevention of symptoms associated with the common cold, particularly those involving nasal congestion and sinus issues, such as "Bi Yuan" (nasal discharge) and "Bi Qiu" (rhinitis). One of the classic formulas containing Xanthium is Cang Er Zi San, which is traditionally used for relieving nasal obstruction and sinusitis, frequently attributed to "wind-cold" invasion in TCM.

The supposed mechanism for its use in colds is the "dispelling of wind and dampness," which in TCM theory relates to alleviating congestion and inflammation of the nasal passages. While there is a strong basis for its use in traditional practice, modern scientific evidence for its efficacy in preventing or treating colds is limited. Some pharmacological studies have shown that Xanthium contains compounds with anti-inflammatory and antimicrobial properties, but robust clinical trials validating its effectiveness for cold prevention in humans are lacking.

Due to its traditional role and some preliminary evidence for symptom relief (rather than prevention), the evidence is rated as 2 out of 5—supported by tradition and limited laboratory research, but without strong clinical validation. Caution is also advised, as Xanthium seeds can be toxic if not properly processed.
