Umckaloabo, also known as Pelargonium sidoides, is an herbal remedy traditionally used in South Africa for respiratory infections. In recent decades, several randomized controlled trials and systematic reviews have investigated its efficacy for treating acute respiratory tract infections, including the common cold—sometimes accompanied by fever. A 2013 Cochrane review concluded that P. sidoides extracts may reduce the duration and severity of symptoms of acute bronchitis and the common cold in adults and children. However, the quality of evidence was considered moderate, with concerns related to study design and potential bias. Some studies showed statistically significant improvements in symptom scores, such as faster resolution of cough, sore throat, and nasal congestion, compared to placebo. The proposed mechanism involves immune-modulating and mild antimicrobial properties of compounds found in the extract.
