Adsorbents, such as activated charcoal and kaolin, have a long history of traditional use in the management of colic and other gastrointestinal disturbances in adults. The rationale behind their application is based on their ability to adsorb gases, toxins, and potentially irritating substances within the gastrointestinal tract, theoretically reducing bloating and discomfort associated with colic. Historical medical texts and pharmacopeias often list adsorbents among remedies for digestive complaints, including colic, especially before the advent of more targeted pharmacological treatments.

However, modern scientific validation for their efficacy in adult colic is limited. While adsorbents are still included in some over-the-counter remedies for nonspecific gastrointestinal symptoms, high-quality clinical trials demonstrating clear benefit in adult colic are lacking. Most available studies are either dated, poorly controlled, or focused on other indications such as poisoning or diarrhea. Contemporary clinical guidelines rarely recommend adsorbents as a primary treatment for adult colic, reflecting the low level of evidence.
