Bacillus coagulans is a spore-forming probiotic that has been studied for its potential benefits in gastrointestinal disorders, including colitis. The scientific evidence supporting its use in colitis—particularly ulcerative colitis and other forms of inflammatory bowel disease (IBD)—is limited but emerging. Several small clinical trials and animal studies suggest that B. coagulans may improve symptoms of colitis through modulation of gut microbiota, enhancement of the intestinal barrier, and anti-inflammatory effects. For example, a randomized controlled trial published in 2014 (Madempudi et al., World J Gastroenterol) demonstrated that supplementation with B. coagulans Unique IS2 improved symptoms and quality of life in patients with irritable bowel syndrome (IBS), a related but distinct condition. Some small pilot studies and preclinical research indicate reductions in colonic inflammation and improved histological scores in animal models of colitis when treated with B. coagulans. However, the quantity and quality of human clinical evidence specifically targeting colitis (such as ulcerative colitis or Crohn’s disease) is modest; most studies are either small-scale, use B. coagulans as part of a multi-strain formulation, or focus on related disorders. Consequently, while there is some scientific rationale and early clinical evidence, B. coagulans cannot yet be considered an evidence-based treatment for colitis. Its use is best described as "promising but unproven," meriting a moderate evidence rating.
