Bromelain, an enzyme extract derived from pineapples, has been explored for its potential benefits in managing colitis, particularly inflammatory bowel diseases such as ulcerative colitis. The scientific rationale for using bromelain centers on its anti-inflammatory and immunomodulatory properties. Preclinical studies, especially in animal models, have shown that bromelain can reduce inflammation and colonic tissue damage by modulating cytokine production, reducing leukocyte migration, and enhancing mucosal healing. For example, several mouse studies (e.g., Hale et al., 2005, Clinical Immunology) demonstrated that orally administered bromelain decreased the severity of chemically-induced colitis, possibly by affecting T-cell and macrophage activity. However, robust clinical evidence in humans is limited. A small number of pilot studies and case reports suggest some symptom improvement in patients with ulcerative colitis, but these are not large, controlled clinical trials. The available human data are not sufficient to draw firm conclusions, and thus bromelain is sometimes used as an adjunct to conventional therapy rather than as a primary treatment. In summary, while there is a scientific rationale and some preclinical evidence for bromelain’s use in colitis, high-quality clinical trials are lacking. Its use is not based on traditional medicine for this indication but rather on emerging scientific investigation, and the overall level of evidence supporting its efficacy in colitis remains low (rated 2 out of 5).
