Immunoglobulin G (IgG) has some scientific basis for use in the treatment of colitis, particularly in certain clinical contexts. IgG is the most abundant type of antibody in the blood and is critical for immune defense. In the context of colitis—especially severe or refractory cases of ulcerative colitis or Crohn’s disease—intravenous immunoglobulin (IVIG), which is a pooled preparation of IgG from multiple donors, has been investigated as a treatment option. The rationale is that IVIG can modulate immune responses, decrease inflammation, and neutralize pathogenic autoantibodies or microbial toxins that may contribute to colitis. Clinical evidence, however, is limited and mostly consists of case reports, small case series, and a few uncontrolled studies. Some reports indicate that IVIG can induce remission in patients with steroid-refractory or treatment-resistant inflammatory bowel disease (IBD), including colitis. However, larger randomized controlled trials are lacking, and current guidelines do not recommend IVIG as a standard therapy for colitis due to insufficient high-quality evidence. Oral IgG preparations (such as serum-derived bovine immunoglobulin) have also been explored in some studies, showing potential benefits by binding and neutralizing microbial antigens and toxins in the gut. Some small trials and observational studies suggest possible improvement in symptoms, but again, the evidence is not robust. In summary, while there is a scientific rationale and some clinical experience supporting the use of IgG for colitis, the overall evidence is weak and not definitive, meriting a low-to-moderate evidence rating.
