Lactobacillus brevis has been explored as a probiotic with potential benefits in the management of colitis, particularly in preclinical studies. Most evidence comes from animal models, where administration of L. brevis strains has shown the ability to modulate gut microbiota, reduce inflammation, and strengthen intestinal barrier function in chemically-induced colitis (such as DSS or TNBS models in mice and rats). Proposed mechanisms include increased production of short-chain fatty acids, upregulation of anti-inflammatory cytokines, and suppression of pro-inflammatory pathways like NF-κB. Some strains, such as L. brevis KB290 and L. brevis 119-2, have been specifically highlighted in animal studies for their effects on decreasing colonic inflammation and promoting mucosal healing.

However, direct evidence in humans is very limited. Only a handful of small clinical studies and case reports have assessed multi-strain probiotic preparations containing L. brevis (often combined with other lactobacilli and bifidobacteria) in individuals with ulcerative colitis or Crohn’s disease, with mixed or modest results. There are no large-scale, high-quality randomized controlled trials that single out L. brevis as an effective monotherapy for colitis in humans.

In summary, while there is promising preclinical data and limited early clinical evidence supporting the potential role of L. brevis for colitis, robust scientific validation in human populations is lacking. Thus, the evidence rating is 2 out of 5, reflecting preliminary support but significant need for further research.
