Sodium butyrate, a short-chain fatty acid (SCFA), is increasingly studied for its potential to support or treat colitis, particularly ulcerative colitis and other forms of inflammatory bowel disease (IBD). Scientific evidence supporting its use comes primarily from preclinical animal studies and a limited number of small human trials. Butyrate is a primary energy source for colonocytes and plays a crucial role in maintaining intestinal barrier function, modulating immune responses, and reducing inflammation.
Animal models of colitis have consistently demonstrated that oral or rectal administration of sodium butyrate can reduce inflammation, improve mucosal healing, and modulate the gut microbiome. In humans, several small clinical trials and pilot studies—often using rectal enemas of sodium butyrate—have reported improvements in symptoms and endoscopic scores in patients with distal ulcerative colitis or proctitis. However, evidence is mixed, and some studies have failed to show significant benefit compared to placebo or standard therapy.
The overall clinical evidence remains limited by small sample sizes, short duration, and variability in formulations and administration routes. There are no large-scale, definitive randomized controlled trials establishing sodium butyrate as a standard therapy for colitis. Nevertheless, the biological rationale and preliminary results provide a scientific basis for its investigation, justifying a moderate evidence rating.
