Streptococcus thermophilus is a probiotic bacterium commonly used in the fermentation of dairy products, particularly yogurt. Its use in supporting or treating colitis, especially ulcerative colitis, is primarily based on scientific research rather than tradition. Several clinical and preclinical studies have explored the effects of S. thermophilus, often in combination with other probiotic strains such as Lactobacillus and Bifidobacterium, on inflammatory bowel diseases (IBD) including colitis.

Evidence suggests that S. thermophilus can help restore intestinal microbial balance and improve mucosal barrier function. In particular, the probiotic mixture known as VSL#3, which contains S. thermophilus among other strains, has shown efficacy in inducing and maintaining remission in patients with mild to moderate ulcerative colitis and in preventing pouchitis after surgery. Mechanistic studies propose that S. thermophilus may modulate immune responses, reduce pro-inflammatory cytokines, and enhance the production of protective short-chain fatty acids in the gut.

However, the strength of the evidence is moderate (rated 3 out of 5), as most clinical trials feature S. thermophilus as part of a multi-strain formulation, making it difficult to isolate its individual effects. While results are promising and have led to some recommendations for probiotic use in certain colitis cases, further large-scale, high-quality trials specifically targeting S. thermophilus are needed to conclusively establish its efficacy for colitis as a standalone therapy.
