Transforming growth factor beta (TGF-β) is a multifunctional cytokine involved in the regulation of immune responses and tissue repair. Its use in the context of colitis, particularly inflammatory bowel disease (IBD) such as ulcerative colitis and Crohn’s disease, is supported by preclinical and some clinical research. TGF-β is known for its anti-inflammatory properties; it suppresses the activation and proliferation of immune cells that drive intestinal inflammation and promotes epithelial cell restitution and mucosal healing. Experimental models of colitis in animals have demonstrated that administration of TGF-β or enhancement of its signaling can reduce inflammation, decrease histological damage, and improve clinical symptoms. In humans, direct administration of TGF-β is not standard practice due to challenges with delivery, potential off-target effects, and fibrogenic (scarring) risk. However, therapies designed to enhance TGF-β signaling, such as the use of TGF-β-rich enteral nutrition formulas (e.g., Modulen IBD), have been explored and are part of some nutritional strategies for pediatric Crohn’s disease. Additionally, altered TGF-β signaling has been implicated in the pathogenesis of IBD, and strategies to restore normal TGF-β activity are under investigation. Overall, while animal and some human studies suggest beneficial effects, the evidence is moderate, and TGF-β-based therapies are not yet routine clinical practice for colitis. Ongoing research aims to clarify its therapeutic potential and safety in this context.
