Adrafinil is a synthetic nootropic compound that is metabolized in the body to modafinil, a well-known wakefulness-promoting agent. Scientific research on adrafinil itself is limited; however, its metabolite, modafinil, has been extensively studied for its effects on alertness, attention, and cognitive performance. Several controlled studies on modafinil demonstrate improvements in measures of attention, vigilance, and executive function, particularly in sleep-deprived or clinically fatigued populations. Modafinil is approved for the treatment of narcolepsy and other sleep disorders, but it is also used off-label for cognitive enhancement and to treat attention deficits.

Adrafinil, as a prodrug, is believed to share these cognitive benefits, although direct evidence is less robust. Most human data regarding adrafinil's cognitive effects are anecdotal or derive from its use in France during the 1980s and 1990s to treat elderly patients with attention and alertness problems. Peer-reviewed studies specifically on adrafinil for poor concentration are sparse, but the shared pharmacological mechanism with modafinil provides a scientific rationale for its use. The overall evidence supports a moderate level of efficacy (3/5), largely extrapolated from modafinil research, rather than from direct, high-quality studies on adrafinil itself.
