Pyridoxal-5-phosphate (P-5-P) is the active form of vitamin B6 and plays a critical role in neurotransmitter synthesis, including the production of serotonin, dopamine, and gamma-aminobutyric acid (GABA), which are essential for normal brain function. Deficiency in vitamin B6 can lead to neurological symptoms, including confusion, irritability, and even seizures in severe cases. Clinical reports and case studies have documented improvement of confusion and cognitive symptoms after repletion of vitamin B6 in deficient individuals. However, the evidence supporting the use of P-5-P specifically for confusion in the general population, or in cases not related to an established deficiency, is limited. Most well-designed clinical trials have not demonstrated a benefit of vitamin B6 supplementation for cognitive function or confusion in healthy adults or in those with dementia. Major guidelines do not recommend routine use of P-5-P for confusion except in the context of deficiency, which can occur in malnutrition, alcoholism, certain genetic disorders, or as a side effect of some medications (e.g., isoniazid). In summary, while scientific evidence clearly shows that P-5-P supplementation resolves confusion caused by vitamin B6 deficiency, there is weak or insufficient evidence to support its use for confusion due to other causes.
