The rationale is that the volatile oils (such as eugenol and cineole) present in bay leaf may exhibit mild expectorant and decongestant properties.

However, scientific evidence supporting bay leaf's efficacy for congestion is limited. A few in vitro and animal studies have demonstrated that certain compounds in bay leaf possess anti-inflammatory and antimicrobial activities, which may theoretically help with respiratory symptoms. Yet, there are no robust clinical trials or systematic reviews that confirm its effectiveness in relieving congestion in humans.
