Elecampane (Inula helenium) has a long history of use in traditional herbal medicine, especially in Europe and Asia, for the treatment of respiratory conditions such as congestion, coughs, and bronchitis. Its roots have been used in folk remedies as an expectorant—an agent that helps expel mucus from the lungs. The traditional use is largely based on its content of inulin, alantolactone, and other sesquiterpene lactones that are thought to contribute to its expectorant and soothing effects on the respiratory tract. Historical texts and herbal compendia frequently reference elecampane as beneficial for “phlegm” and chest complaints.

Regarding scientific evidence, modern research is limited and mainly consists of in vitro (test tube) and animal studies. Some studies have shown that alantolactone possesses anti-inflammatory and antimicrobial properties, which could theoretically contribute to easing respiratory symptoms. However, robust clinical trials in humans are lacking, and there is insufficient scientific validation to confirm its efficacy for congestion. Thus, its use remains primarily justified by tradition rather than solid scientific evidence.
