Bicarbonate (commonly in the form of sodium bicarbonate) has been traditionally used as a component in saline nasal irrigation solutions for managing sinus congestion. The primary rationale is that adding bicarbonate to saline makes the solution more alkaline, which may help soothe irritated nasal mucosa and improve mucus clearance. While saline irrigation itself has scientific support for reducing nasal congestion and improving sinus health, the specific addition of bicarbonate is based more on tradition and anecdotal experience than robust clinical evidence. Some small studies and clinical guidelines suggest that alkaline solutions (saline with added bicarbonate) may be slightly more comfortable or effective for certain patients, especially those with sensitive nasal passages or after nasal surgery, but high-quality, large-scale studies directly comparing plain saline to bicarbonate-containing solutions are lacking.
