Isatis (commonly referring to Isatis tinctoria or Isatis indigotica) has a long history of use in Traditional Chinese Medicine (TCM). In TCM, Isatis root (ban lan gen) and leaf (da qing ye) are used for their purported "heat-clearing" and "detoxifying" properties. Traditionally, these herbs are used to treat symptoms associated with "wind-heat" invasions, which may include sore throat, fever, and sometimes congestion related to upper respiratory tract infections. However, the primary indications in traditional texts are for viral and febrile illnesses, not specifically sinus congestion.

Modern scientific evidence supporting Isatis for sinus congestion is limited and mainly extrapolated from studies on its antiviral and anti-inflammatory activities. Some in vitro and animal studies have shown that extracts of Isatis possess anti-inflammatory effects and may inhibit pathogens relevant to respiratory infections. However, clinical studies directly evaluating Isatis for sinus congestion are lacking. Most traditional and modern uses focus on its broader effects on respiratory tract infections rather than specifically targeting sinus congestion. As a result, while Isatis is sometimes used in multi-herb formulations for upper respiratory symptoms in traditional contexts, scientific evidence supporting its use for sinus congestion specifically is weak.
