Menthol has a long history of traditional use for the relief of nasal and sinus congestion. It is a common ingredient in over-the-counter remedies like inhalers, rubs (such as Vicks VapoRub), and lozenges. The characteristic cooling sensation menthol imparts is due to its action on cold-sensitive receptors in the nasal mucosa (TRPM8 receptors), which creates a subjective feeling of increased airflow and "clearing" of nasal passages. However, scientific studies have shown that while menthol can make people feel as if they are breathing more easily, it does not actually decrease nasal airway resistance or improve objective measures of airflow in the sinuses. Thus, its use is primarily justified by traditional use and the subjective relief it provides, rather than by robust scientific evidence demonstrating true decongestant effects. The overall scientific evidence supporting menthol’s efficacy for treating sinus congestion is limited, and the benefit is considered primarily perceptual rather than physiological.
