Arginine orotate has been investigated for use in supporting congestive heart failure (CHF), with some degree of scientific rationale and limited clinical research. The compound combines L-arginine, an amino acid precursor to nitric oxide (NO), and orotic acid, a molecule involved in pyrimidine synthesis. Nitric oxide is known to play a role in vascular tone and cardiac function. A small number of clinical studies, mostly from the 1970s and 1980s, suggest that arginine orotate supplementation may improve cardiac function, exercise tolerance, and symptoms in patients with heart failure. For example, a 1976 study by Milner and colleagues reported improved cardiac performance in CHF patients receiving arginine orotate versus placebo. The proposed mechanism involves improved endothelial function and myocardial energy metabolism.

However, the number and quality of these studies are limited: most trials are small, short-term, and lack rigorous contemporary methodology. Since then, larger-scale studies have not confirmed these benefits, and arginine orotate is not included in modern treatment guidelines for CHF. Nevertheless, the use of L-arginine (not necessarily the orotate salt) continues to be explored for cardiovascular support, but with mixed results and concerns about safety in advanced heart failure.

In summary, there is modest scientific evidence from early clinical research supporting arginine orotate for CHF, but the evidence base is weak (rated 2/5), and the compound is not widely recommended in current evidence-based practice.
