Ligustrazin (tetramethylpyrazine) is an active compound extracted from the traditional Chinese medicinal herb Ligusticum chuanxiong. Its use in treating or supporting congestive heart failure (CHF) is primarily based on traditional Chinese medicine (TCM) practices, where it has been used for centuries to promote blood circulation and alleviate cardiovascular symptoms. In TCM, Ligustrazin-containing formulas are believed to "invigorate blood," dispel blood stasis, and improve heart function.

Scientific research on Ligustrazin's efficacy in CHF is limited and largely consists of small-scale clinical studies and animal experiments, mostly published in Chinese literature. Some studies suggest that Ligustrazin has vasodilatory, anti-inflammatory, and anti-platelet aggregation effects, which could theoretically benefit CHF patients by improving microcirculation and reducing cardiac workload. However, the methodological quality of these studies is generally low, with issues such as small sample sizes, lack of rigorous controls, and limited replication in international journals.

Systematic reviews and meta-analyses have concluded that while there is some evidence suggesting benefit, the current data are insufficient for firm scientific validation. As a result, Ligustrazin’s use for CHF remains rooted in tradition, with only modest supporting evidence from modern scientific studies. More rigorous clinical trials are needed to establish its safety and efficacy for this indication.
