Phthalides are a class of chemical compounds found primarily in certain plants, notably in celery (Apium graveolens) and the traditional Chinese medicinal herb Ligusticum chuanxiong (Chuanxiong). In Traditional Chinese Medicine (TCM), extracts containing phthalides have been used to promote cardiovascular health, including for symptoms associated with congestive heart failure (CHF). The traditional use is grounded in TCM theory, where herbs like Chuanxiong are believed to "invigorate blood circulation" and "remove blood stasis," and are often prescribed for conditions involving chest pain, palpitations, or heart weakness.

Scientific evidence supporting the use of phthalides specifically for CHF is limited and mostly preclinical. Some studies have shown that phthalides may have vasodilatory, anti-inflammatory, and anti-platelet aggregation effects, which could theoretically benefit cardiovascular health. However, well-designed clinical trials in humans with CHF are lacking. The traditional use persists in some Asian countries, but mainstream medical guidelines do not endorse phthalides or related herbal extracts for CHF management. Thus, the justification for their use is primarily traditional, with some supporting preclinical evidence but insufficient clinical validation.
