Puerarin is an isoflavone derived from the root of Pueraria lobata (kudzu), a plant traditionally used in Chinese medicine. In recent years, scientific interest has grown regarding its potential cardiovascular benefits, including its use in supporting or treating Congestive Heart Failure (CHF). Several preclinical studies and a limited number of clinical trials have explored the effects of puerarin on heart function. Mechanistically, puerarin is believed to exert its benefits through vasodilation, antioxidant effects, anti-inflammatory properties, and by improving myocardial energy metabolism. Animal models have shown that puerarin can reduce myocardial injury and improve cardiac output in heart failure settings. Some small-scale clinical studies in China have reported that puerarin, often administered intravenously, may improve symptoms and cardiac function in patients with CHF when used as an adjunct to standard therapy. However, these studies tend to have methodological limitations, such as small sample sizes, lack of blinding, and short duration. Systematic reviews recognize these limitations and generally conclude that while puerarin shows promise, the quality of evidence is low to moderate and further high-quality, large-scale randomized controlled trials are needed to confirm efficacy and safety. In summary, the use of puerarin for CHF has some preliminary scientific backing, but current evidence remains insufficiently robust to recommend its routine use outside of research settings.
