Salvianolic acid, a major active component found in the traditional Chinese medicinal herb Salvia miltiorrhiza (Danshen), has been studied for its cardiovascular effects, including its potential role in the management of congestive heart failure (CHF). Preclinical studies (in vitro and in animal models) suggest that salvianolic acid possesses antioxidant, anti-inflammatory, vasodilatory, and anti-fibrotic properties. These actions may help protect cardiac tissue, improve myocardial function, and decrease pathological remodeling associated with heart failure. Some small-scale clinical trials and meta-analyses conducted in China have reported improvements in cardiac function parameters (such as ejection fraction and cardiac output) when salvianolic acid or Danshen extracts were used as adjunct therapy in patients with CHF.

However, the quality of clinical evidence is limited by small sample sizes, lack of rigorous randomized controlled trials, and variability in formulations and dosing. Most studies have been published in Chinese journals and may not meet international standards for clinical research. There is currently no large, high-quality, placebo-controlled trial establishing the efficacy or safety of salvianolic acid for CHF in Western medical practice. As a result, while there is some scientific investigation supporting its use, the evidence is preliminary and not sufficient to warrant strong recommendations for its use outside of adjunctive or traditional settings.
