Cape Aloe (Aloe ferox) is used to treat constipation primarily due to its well-established laxative properties. The dried latex of Cape Aloe contains anthraquinone glycosides (such as aloin), which are metabolized in the colon by bacteria to active compounds that stimulate peristalsis and increase water secretion in the bowel, resulting in a laxative effect. This mechanism is similar to that of senna and cascara sagrada, other well-known stimulant laxatives.
There is scientific validation for the use of Cape Aloe in treating constipation in adults. Multiple in vitro and animal studies, as well as some human clinical trials, have demonstrated its efficacy as a short-term remedy for occasional constipation. For instance, a randomized controlled trial comparing Aloe, cascara, and psyllium in adults with chronic constipation found that Aloe was effective at increasing stool frequency and softening stool consistency. However, the number of high-quality, large-scale human trials is limited, and most evidence comes from traditional use, case reports, and small studies.
Regulatory agencies such as the European Medicines Agency (EMA) recognize the use of Aloe latex preparations for short-term relief of occasional constipation but caution against long-term use due to the risk of adverse effects, such as electrolyte imbalances and dependence. In summary, there is scientific evidence supporting its short-term use for constipation, though safety concerns warrant caution.
