Emodin is an anthraquinone compound found in several plants, including rhubarb (Rheum species) and senna (Senna alexandrina), both of which have a long history of use as herbal laxatives. The laxative effect of emodin is primarily due to its ability to stimulate intestinal peristalsis and inhibit water reabsorption in the colon, resulting in softer stools and increased bowel movements. Several in vitro and in vivo studies have demonstrated emodin’s capacity to promote bowel motility and its mechanism of action involves irritation of the intestinal mucosa, leading to enhanced secretion and reduced absorption of water and electrolytes in the colon. Clinical evidence is less robust, but preparations containing emodin-rich plants, such as senna and rhubarb, are widely recognized and approved for short-term relief of constipation in adults by regulatory authorities in multiple countries, including the FDA and EMA. Emodin in its pure form is less commonly used than whole plant extracts, and most clinical research has focused on the herbal preparations rather than isolated emodin. However, the efficacy of emodin as a laxative constituent within these botanicals is well established. Side effects such as abdominal cramping and potential for dependency with prolonged use are noted. Overall, the use of emodin for constipation is supported by both pharmacological studies and historical usage, though direct clinical evidence for isolated emodin is moderate.
