Synopsis

Source of validity: Traditional
Rating (out of 5): 2

Alder Buckthorn (Frangula alnus) has been traditionally used as a herbal remedy for constipation, including in children. The medicinal part of the plant is the dried bark, which contains anthraquinone glycosides. These compounds are known to have a laxative effect by stimulating peristalsis in the colon and increasing water content in the stool. Historical records indicate that both Alder Buckthorn and the related species, Cascara Sagrada, have been included in European pharmacopeias and used for centuries as natural laxatives. The bark must be aged for at least a year or heat-treated before use to reduce its emetic and irritating properties.

Despite the strong traditional use, modern scientific studies specifically evaluating Alder Buckthorn in children are scarce. Most evidence is extrapolated from broader studies on anthraquinone-containing plants and their known mechanism of action on the digestive tract. Safety concerns exist, particularly with chronic use or high doses, as anthraquinone laxatives can cause abdominal cramps, diarrhea, and, with prolonged use, electrolyte imbalances. As a result, its use in children is generally not recommended in current medical practice, and it is not endorsed by major pediatric guidelines.

In summary, Alder Buckthorn’s use for constipation in children is primarily rooted in traditional herbal medicine, with moderate evidence for efficacy based on its pharmacological components, but limited direct clinical research, especially for pediatric populations.
