Bacillus species, including Bacillus clausii, Bacillus coagulans, and Bacillus subtilis, are spore-forming probiotic bacteria that have been studied for their effects on gastrointestinal health, including constipation in children. Several small clinical trials and observational studies have explored the use of Bacillus-based probiotics for pediatric constipation. Some studies suggest that these probiotics can modestly improve stool frequency and consistency by modulating the gut microbiota, increasing short-chain fatty acid production, and improving intestinal motility. However, the overall quality of evidence is low to moderate, with limitations including small sample sizes, heterogeneity in probiotic strains and formulations used, and short study durations. A 2022 review notes that while Bacillus coagulans and Bacillus clausii are safe and may provide some benefit in functional gastrointestinal disorders, the evidence specifically for childhood constipation is limited and not robust enough to make strong recommendations. No major guidelines recommend Bacillus probiotics as a first-line therapy for pediatric constipation, and more rigorous, large-scale studies are needed. In summary, while there is emerging scientific interest and some preliminary clinical data suggesting a possible benefit, the evidence base is currently weak, supporting a low evidence rating.
