Fructooligosaccharides (FOS) are short-chain prebiotic fibers that are known to promote the growth of beneficial gut bacteria, particularly bifidobacteria and lactobacilli. The use of FOS for treating or supporting childhood constipation is primarily supported by scientific evidence rather than tradition. Several randomized controlled trials and systematic reviews have investigated the efficacy of FOS and other prebiotics in alleviating constipation in children. These studies generally show that FOS supplementation can increase stool frequency, improve stool consistency, and reduce symptoms of constipation. The proposed mechanism involves FOS reaching the colon undigested, where it undergoes fermentation by gut microbiota, leading to increased production of short-chain fatty acids that stimulate bowel movements and soften stool by increasing water content. A systematic review published in 2019 in the journal Pediatrics found that prebiotic supplementation, including FOS, was associated with a modest but statistically significant improvement in stool frequency in children with functional constipation. However, the overall quality of evidence is moderate, and not all studies report positive results. Some studies are limited by small sample sizes and short durations. Side effects are generally mild, mostly limited to transient bloating or gas. In summary, while FOS is not a first-line therapy for childhood constipation, there is moderate scientific evidence supporting its use as an adjunctive therapy, especially for mild cases or as a preventive measure in children at risk of constipation.
