Guar gum is a soluble dietary fiber derived from the seeds of the guar plant (Cyamopsis tetragonoloba). Its use in supporting or treating constipation, including in children, is primarily justified by scientific evidence. Guar gum increases stool bulk and water content, which can help stimulate bowel movements. Several randomized controlled trials have evaluated partially hydrolyzed guar gum (PHGG) in pediatric populations. For example, a 2012 study published in the journal Pediatrics found that PHGG supplementation significantly increased stool frequency and improved stool consistency in constipated children compared to placebo. Other studies have reported similar outcomes, with improvements in both frequency and the ease of defecation, and minimal side effects.

The mechanism of action is attributed to its prebiotic effects and ability to retain water, forming a gel-like substance that facilitates stool passage through the colon. Guar gum is generally well tolerated, with few reported adverse effects, although excessive intake can sometimes cause bloating or gas. Guidelines such as those from the North American Society for Pediatric Gastroenterology, Hepatology and Nutrition (NASPGHAN) mention fiber supplementation, including guar gum, as a potential adjunct to standard therapies for pediatric constipation. However, the overall quality of evidence is moderate, and not all children respond equally. Further large-scale, high-quality studies are warranted, but current data support its use as a fiber supplement for childhood constipation.
