Lactobacillus species, as mixed probiotic preparations, have been investigated for their potential to alleviate constipation in children. The rationale for their use is based on the hypothesis that modifying the intestinal microbiota may improve bowel function. Several randomized controlled trials (RCTs) and systematic reviews have assessed the efficacy of probiotics, including mixed Lactobacillus strains, for pediatric constipation.

Evidence is mixed but somewhat promising. A 2017 systematic review (Coccorullo et al., 2017) found that some specific strains of Lactobacillus, especially when used in combination with other probiotics (such as Bifidobacterium), may increase stool frequency in constipated children compared to placebo. However, the clinical significance of these findings is debated, and not all studies demonstrate benefit. For example, a 2015 Cochrane review (Guerra et al.) concluded that the overall evidence is inconclusive, with only modest improvements in stool frequency and no clear effect on stool consistency or abdominal pain. Importantly, the benefit seems to depend on the specific strains and combinations used, as well as the duration of therapy.

Guidelines, such as those from the North American Society for Pediatric Gastroenterology, Hepatology and Nutrition (NASPGHAN), do not currently recommend probiotics as first-line therapy for constipation in children, citing insufficient high-quality evidence. In summary, there is some scientific support for the use of mixed Lactobacillus preparations in pediatric constipation, but the effect size is generally small and inconsistent, warranting a moderate evidence rating.
