Ligustilides are bioactive compounds primarily found in plants such as Angelica sinensis (Dong Quai) and Ligusticum chuanxiong, both of which are important herbs in Traditional Chinese Medicine (TCM). Traditionally, these herbs have been used to address a variety of ailments, including digestive issues such as constipation, especially in children. The traditional use is based on the belief that these herbs can help "move the blood" and "regulate Qi," which in TCM theory can alleviate stagnation thought to contribute to constipation. However, while there are historical records and longstanding usage patterns supporting the use of these herbs (and thus ligustilides) for constipation, particularly in pediatric formulas, robust scientific validation is lacking. Modern pharmacological studies on ligustilides have primarily focused on their anti-inflammatory, neuroprotective, and vasodilatory effects, with very limited direct research on their effects on bowel motility or constipation, especially in children. Therefore, the use of ligustilides for pediatric constipation is justified primarily by tradition rather than strong scientific evidence.
