Oligosaccharides, particularly prebiotic types such as fructooligosaccharides (FOS) and galactooligosaccharides (GOS), have scientific support for their use in managing constipation in children. These non-digestible carbohydrates promote the growth of beneficial gut bacteria, such as Bifidobacteria and Lactobacilli, which can enhance stool frequency and consistency. Several randomized controlled trials (RCTs) have evaluated the effectiveness of oligosaccharides in pediatric constipation. For example, studies published in peer-reviewed journals have reported that supplementation with GOS and FOS in infant formulas leads to softer stools and increased stool frequency, closely resembling the stool patterns of breastfed infants. In addition, a 2017 systematic review and meta-analysis found that prebiotic supplementation improved stool frequency in children with constipation compared to placebo, although the magnitude of the effect varied between studies.

The mechanism is thought to involve fermentation of oligosaccharides in the colon, resulting in the production of short-chain fatty acids, which can stimulate colonic motility and improve stool water content. While not all children respond to prebiotic supplementation, and the quality of evidence varies, oligosaccharides are considered a safe and viable adjunct therapy for mild to moderate constipation in children. However, more high-quality, large-scale studies are needed to determine optimal dosing and long-term efficacy.
