Ricinoleic acid is the primary active component of castor oil, which has a long history of use as a stimulant laxative for the treatment of constipation, including in children. The mechanism of action is well-characterized: after oral ingestion, ricinoleic acid is released by intestinal lipases and stimulates intestinal peristalsis by activating EP3 prostanoid receptors on smooth muscle cells, increasing motility and promoting bowel movements. Several clinical studies and reviews support the efficacy of castor oil as a laxative in adults. However, the direct evidence specifically in children is limited, and concerns about safety, such as potential for dehydration, electrolyte imbalance, and cramping, have led to decreased use in pediatric populations in modern clinical practice. Guidelines from pediatric organizations generally recommend other, safer laxatives for children, but castor oil (and thus ricinoleic acid) is still sometimes used in traditional and some clinical settings, particularly where other options are limited. In summary, the scientific mechanism is validated and there is moderate evidence for efficacy in adults, but pediatric use is less supported and may carry additional risks.
