Leucine alpha-ketoglutarate (Leu-AKG) is a compound used in some nutritional supplements, particularly those aimed at supporting muscle recovery and protein synthesis during periods of convalescence (recovery from illness, surgery, or trauma). The rationale for its use is based on the roles of its two components: leucine, a branched-chain amino acid (BCAA) known to stimulate muscle protein synthesis via mTOR signaling, and alpha-ketoglutarate, an intermediate in the Krebs cycle that may support cellular energy metabolism and nitrogen balance. Some clinical and preclinical studies suggest that supplementation with leucine or leucine-enriched formulations can help attenuate muscle loss and promote recovery in catabolic states, including during convalescence. There is also some evidence that alpha-ketoglutarate may help reduce protein breakdown and improve wound healing in critically ill patients. However, direct studies specifically on Leu-AKG in human convalescence are limited, and much of the evidence comes from studies on the individual components or on similar alpha-keto acid/amino acid formulations. Therefore, while there is a plausible scientific rationale and some supporting data, robust clinical trials directly validating the efficacy of leucine alpha-ketoglutarate for convalescence are lacking, and the strength of evidence is moderate at best.
