Glycyrrhiza, commonly known as licorice root, has a long history of use in traditional medicine systems, particularly in Traditional Chinese Medicine (TCM) and Ayurveda. It has been used to support adrenal function and manage stress-related fatigue, which are related to cortisol regulation in the body. The traditional rationale is that Glycyrrhiza tonifies the adrenal glands and helps the body adapt to stress, thereby supporting what is now understood as the "cortisol system." Some traditional formulas incorporate licorice to harmonize the effects of other herbs and to provide a mild adaptogenic effect.

Scientifically, Glycyrrhiza contains glycyrrhizin, a compound that inhibits the enzyme 11β-hydroxysteroid dehydrogenase type 2 (11β-HSD2). This enzyme converts active cortisol to its inactive form, cortisone. By inhibiting this enzyme, licorice root can increase circulating levels of cortisol. Most uses are extrapolated from its known biochemical effects rather than direct research on clinical endpoints related to stress or adrenal health. Overall, the use of Glycyrrhiza for supporting the cortisol body system is primarily justified by traditional use, with some mechanistic scientific rationale but limited clinical validation.
