Rating (out of 5): 4

Malabar Nut Tree Leaf Extract, derived from Justicia adhatoda (also known as Vasaka), has a long history of use in traditional Ayurvedic and Unani medicine for the treatment of respiratory conditions, particularly coughs associated with excessive mucus (sometimes termed "damp" cough). Modern scientific studies have validated several of these traditional uses. The primary active compound, vasicine, exhibits bronchodilatory, expectorant, and antitussive properties. Multiple in vitro and animal studies have demonstrated that extracts from Malabar Nut leaves can help clear airway mucus and reduce cough frequency by relaxing bronchial muscles and thinning mucus secretions. Some clinical trials have also reported symptomatic improvement in patients with productive cough and chronic bronchitis when using Vasaka preparations, although the sample sizes are often small.

Systematic reviews and pharmacological studies suggest that the plant’s efficacy is due to a combination of its expectorant properties, ability to inhibit histamine-induced bronchoconstriction, and mild antimicrobial effects. Regulatory bodies in several countries recognize Vasaka as a component in over-the-counter cough syrups. However, more large-scale, high-quality clinical trials are needed to fully establish dosage and efficacy parameters. Overall, Justicia adhatoda leaf extract is scientifically supported for use in managing productive (damp) coughs, with a moderate to strong evidence base.
