Costus (Saussurea costus, also known as "Kuth") has a long history of use in traditional medicine systems, particularly Ayurveda, Unani, and Traditional Chinese Medicine, for respiratory ailments including cough. Classical Ayurvedic texts describe Costus root as an ingredient in various formulations for chronic cough, asthma, and bronchitis. The root is believed to possess expectorant, anti-inflammatory, and antimicrobial properties, which are thought to help relieve cough and associated symptoms by reducing airway irritation and supporting mucus expulsion. Unani practitioners have similarly used Costus for chest congestion and cough. Scientific studies on Costus for cough are limited, but some laboratory research indicates the plant contains bioactive compounds such as sesquiterpene lactones (e.g., costunolide and dehydrocostus lactone) that may exhibit anti-inflammatory and antimicrobial effects. However, direct clinical trials demonstrating efficacy specifically for cough relief in humans are lacking or very limited. As a result, the main basis for its use in treating cough remains traditional and historical practice rather than robust scientific validation. Care should be taken with its use, as Costus is considered endangered in some regions and may interact with certain medications.
