Marshmallow (Althaea officinalis) root has a long history of traditional use as an herbal remedy for cough and throat irritation. The plant contains high levels of mucilage, a gelatinous substance that coats mucous membranes. Historically, marshmallow root teas, syrups, and lozenges have been used in Europe and the Middle East to soothe sore throats and calm coughs, particularly dry or irritating coughs. The mucilage is thought to form a protective layer over the throat, reducing irritation and suppressing the cough reflex.

Scientific research on marshmallow for cough is limited, but a few small studies and laboratory experiments support its demulcent (soothing) properties. For example, in vitro studies have shown that marshmallow extracts increase the secretion of mucus, which could help relieve irritation. However, there is a lack of large, well-controlled clinical trials in humans directly evaluating its effectiveness for cough. The European Medicines Agency (EMA) recognizes marshmallow root as a traditional herbal medicinal product for the relief of irritation of the oral or pharyngeal mucosa and associated dry cough, based on its longstanding use.
