Buchu (typically referring to species such as Agathosma betulina or Agathosma crenulata) is primarily used in traditional South African herbal medicine. Its main historical applications have been for urinary tract disorders, kidney issues, and as a diuretic. There is limited evidence in the ethnobotanical literature that buchu was also used as a remedy for coughs, including spastic or dry coughs, by some indigenous communities and early settlers. This use, however, is not widespread in modern herbal medicine references. The proposed rationale was often based on the aromatic volatile oils in buchu leaves, which were thought to have a soothing effect on mucous membranes and mild antiseptic properties.

Scientific evidence for buchu's efficacy in treating or supporting spastic cough is essentially absent. No clinical trials or modern pharmacological studies have specifically validated its use for cough suppression or as an antispasmodic for the respiratory tract.
