Glycyrrhizin, a primary active compound found in licorice root (Glycyrrhiza glabra), has a long-standing history of use in traditional medicine systems, particularly in Traditional Chinese Medicine (TCM) and Ayurveda, for alleviating coughs, including spastic or dry coughs. Traditionally, licorice root is considered a demulcent and expectorant, believed to soothe irritated mucous membranes in the respiratory tract and promote the expulsion of mucus. Historical texts and pharmacopeias routinely list licorice as an ingredient in herbal cough remedies.
Scientific studies have identified glycyrrhizin as having anti-inflammatory, antiviral, and some antitussive (cough-suppressing) properties. However, clinical evidence specifically supporting its efficacy in treating spastic cough in humans is limited and mostly anecdotal or based on animal studies. Some laboratory experiments suggest glycyrrhizin can reduce inflammation and irritation in the airway, potentially offering symptomatic relief. Nonetheless, high-quality, large-scale clinical trials validating its effectiveness for spastic cough are lacking.
Therefore, the use of glycyrrhizin for cough (spastic) is primarily based on traditional knowledge rather than robust scientific validation. Caution should also be exercised due to potential side effects of glycyrrhizin, such as hypertension and hypokalemia, especially with prolonged use or high doses.
